As I requested on phone, I am in need of an experience certificate for the purpose of enrolling for a Quality management course.
I would appreciate it very much if you could issue me an experience certificate outlining the following:
- my position
- a brief description of responsibilities
- the date I joined the company

Try and be specific in who the “Sir” is. If you don’t know, put down “To whom it may concern.”
When you said, as requested by phone, go ahead and say what day you requested it. Say, “As I requested BY phone on January 12, 2009″ or give an approximate date.
Take out the “very much” in the I would appreciate line.
Very good letter.

Close with “Thanks for your kind help” it works wonderfully on people
